You are looking at a Medal instituted March 12, 1881 in memory of Alexander II. This Silver Medal was awarded to all individuals who were in the company of Emperor Alexander II during the attempt on his life March 1, 1881.
The obverse of the silver Medal bears the cipher of Tsar Alexander II topped by the Imperial crown. The reverse of the Medal is plain, except for the inscription signifying: "First of March 1881".
